武汉协和董念国团队全国首创采用腋、颈微创介入方式建立体外心室辅助循环支持

近期,华中科技大学同济医学院附属协和医院心外科董念国教授团队运用与苏州心擎医疗技术有限公司联合研发的体外短中期心室辅助装置,成功通过颈静脉及腋动脉微创介入方式连接体外磁悬浮人工心脏的方式完成循环支持,极大的减少患者创伤,术后恢复快,同时不会影响下肢行动,帮助患者可以下地活动,平稳等待心脏移植。该手术的成功实施为国内首创,标志着短中期体外磁悬浮心室辅助装置的置入手术实现全面微创化,具有突破性意义。 患者为56岁男性,9年前无明显诱因出现胸闷喘气,诊断考虑为扩张型心肌病,心力衰竭。2月前无明显诱因出现胸闷喘气加重,治疗效果欠佳,血压仍旧偏低,频发室性心率失常。家属为求进一步诊治,转至我院。转至我院后,因病情危重,收治后即转入ICU,出现慢性心衰急性加重情况。患者经诊断患有扩张型心肌病、难治性心力衰竭、心源性休克、室性心律失常、多脏器损伤等。患者病情危重,暂无供心,考虑行机械循环辅助。 经董念国教授团队评估并经家属同意,决定置入心擎医疗体外磁悬浮人工心脏进行循环支持。采用微创介入方式进行右侧颈静脉-右侧腋动脉插管建立左房-体外人工心脏-腋动脉循环辅助。 本次手术由陈澍等教授经董念国教授的指导下,在心内科、超声科、麻醉科、体循等团队支持中完成。 手术先经右侧股静脉穿刺置入导丝,行右心导管检查。随后在超声及DSA引导下进行房间隔穿刺后置入导丝,并延股静脉导丝在右心房内置入圈套器。经右侧颈静脉送入导管,并由圈套器捕获,延股静脉导丝一起送入左心房内。随后延颈静脉导丝入路置入引流管,建立经颈静脉、上腔静脉、右房、房间隔至左房的引流路径。右锁骨下切口游离腋动脉,端侧吻合人工血管,从而建立左心房-体外人工心脏-腋动脉循环辅助,进而连接心擎医疗体外磁悬浮人工心脏后开泵,经李平教授的调整下,人工心脏平稳运转,患者情况稳定,手术顺利结束后转至ICU。 患者转入ICU后,术后6小时清醒,11小时撤除气管插管,14小时可以进食;因采用腋部及颈部的循环支持插管,不会影响下肢行动,术后第二天即可下床行走进行锻炼。目前体外 磁悬浮人工心脏 支持稳定,患者目前平稳等待供心中。 患者为24岁男性,9年前无明显诱因出现发热后胸闷,不能平躺,当地医院检查提示为心脏扩大。4年前胸闷喘气症状发作频繁。2年前进行心脏移植等待登记。后为求进一步心脏移植,遂来我院。患者经诊断患有扩张型心肌病,NYHA IV级,终末期心力衰竭,肺动脉高压,左室舒张功能减低(III级),二尖瓣重度关闭不全等。入院期间,患者心衰症状进行性加重,后转至ICU。患者低心排,强心、利尿等治疗改善不佳,血压持续在60~70/40~50 mmHg,心率82 bpm,随时可能发作多器官衰竭、恶性心律失常、心脏骤停甚至死亡等风险,而供心仍在等待期。
